This page will be your "one stop shop" for accessing and downloading activity request forms, reimbursement forms, and other pertinent documents related to church-specific activities and functions. Activity Request Form
Purpose: The purpose of the EBC Activity Request Form is to support the booking of EBC facilities to support your community event. Please turn download, fill-out, and return the form to the EBC trustee ministry or the EBC Office Administrator at least 4 weeks before your activity/event, to ensure that there is sufficient time to properly plan, advertise, and/or purchase the supplies that your activity will need.
Financial Voucher Form
Purpose: The purpose of the EBC Financial Voucher Form is to provide a mechanism whereby individuals or ministries can be reimbursed by the church for expenses incurred for an APPROVED church-related event. All financial receipts accumulated are required to be submitted along with the financial voucher. No exceptions.
Voucher Reconciliation Form
Purpose: The purpose of the EBC Voucher Reconciliation Form is for reconciliation of expenses incurred during the execution of an APPROVED church-related event whereby individuals or ministries can be reimbursed by the church or need to repay monies to the church as a result of advancement of funds to support the approved event. All financial receipts accumulated are required to be submitted along with the reconciliation form. No exceptions.
Special Deposit Form
Purpose: The purpose of the EBC Special Deposit Form is to allow inviduals, ministries, or organizations to submit funds to EBC in support of an church-related event or activity.
